Please read these Terms of Use (“Terms”) before using the Campus Mental Health Action Tracker (“the Tracker”). By using the Tracker, you agree to comply with these Terms. If you disagree with these terms, you may not access the Tracker.
Prohibited Conduct
The purpose of the Tracker is to encourage collaborative assessment. To facilitate this process, all responses will be shared with the campus organizational representative. Individual responses will be de-identified and included in the composite scores for your campus.
To ensure data integrity, you should not share your account or password with anyone, nor share the information you obtain through the Tracker with anyone other than those who are also enrolled in the process.
Termination
The Social Research and Demonstration Corporation (SRDC) or your organizational representative may suspend or delete your account at any time without prior notice or liability, for any reason, including without limitation if you breach the Terms.
Limitation of Liability
Your use of the Tracker is at your own risk. SRDC and your institution are not liable, directly or indirectly, with any direct, indirect, special, reputational or consequential loss or damage incurred by any user in connection with this Tracker or in connection with the use, inability to use, or results of the use of the Tracker.
